SUBJECT: QE2Q - Official UC DAVIS Questionnaire
The survey login is at
http://www.sariweb.ucdavis.edu/Survey/Survey.cfm
The survey is restricted to UCD undergraduate students and
requires the combination of your first initial + first two letters of last name
AND EITHER the last six numbers of your student ID OR your survey number.
Your survey number is ____.
Dear Student:
This is an important opportunity to tell the faculty and
administration about the quality of your undergraduate educational experience
at UC Davis. This is an official University questionnaire administered by
Student Affairs Research & Information and the data collected will be used
to improve both the academic and social experience of current and future
undergraduates at UC Davis. In addition, participating students are included in
a series of five weekly drawings to award at total of $2,500. Your chance of
winning is better if you enter early because you will be included in all
drawings after that date.
A summary report of results will be sent to your academic
department and will be made available to you at http://www.sariweb.ucdavis.edu/
by fall quarter. Because the results will be summarized by major and reported
to academic departments, all UC Davis regular degree-seeking undergraduate
students have been included and every response is critically important.
It should take 10-15 minutes to complete this questionnaire
of about 75 items. The study uses five survey forms to minimize the effort
asked of any one student and issues that you consider important that are not
addressed on your form are very likely addressed on one of the other forms.
Which of the five versions you receive is random.
Your reply will be held in confidence and will not be
individually recognizable in reports produced from these results. More information
about the survey and Student Affairs Research and Information's confidentiality
policy is available at http://www.sariweb.ucdavis.edu/Survey/Survey.cfm
If you are unable to participate due to physical circumstances please contact the Director of Student Affairs Research and Information, Dr. Steve Chatman, at Chatman@
Sincerely,
Carol Wall
Vice Chancellor for Student Affairs
